VITT is mediated by platelet-activating antibodies against the highly cationic protein platelet factor 4 (PF4).\u00a0 The underlying inciting antigen trigger and immunopathogenesis remained unknown.\u00a0 Here, investigators used antibody proteomics to determine the amino acid sequences of anti-PF4 antibodies from 21 patients with VITT and sequenced the genes encoding the immunoglobulin light-chain hypervariable region from 100 patients with VITT. To identify an adenoviral trigger, they used the antigen-binding fingerprints of anti-PF4 and anti-adenovirus protein antibodies to identify a shared serum clonotype and subsequently used adenovirus protein peptides and recombinant anti-PF4 VITT antibodies to map the mimicking linear epitope. Genomic and proteomic profiling of VITT antibodies revealed a shared immunoglobulin light-chain allele, IGLV3-21*02 or *03, harboring a critical somatic hypermutation, K31E. Only antibodies purified against adenoviral core protein VII (pVII) contained anti-PF4 species matching the VITT fingerprint; antibodies against intact virions or other adenoviral proteins did not. Cross-reactive IgGs were mapped to a basic linear epitope on pVII. A pathogenic anti-PF4 VITT antibody, back-mutated to germline (K31), lost its prothrombotic activity in vitro and in vivo and preferentially bound pVII, a finding that directly supported the role of the hypermutation in the antigenic shift from adenovirus pVII to PF4. The results indicate that VITT occurs when, in persons with immunoglobulin light-chain allele IGLV3-21*02 or *03, a specific somatic hypermutation develops that affects antibodies that recognize a specific epitope on the adenoviral core protein pVII, which results in misdirection of antibody targeting toward PF4. \n\t<a href=\"https:\/\/publications.ersnet.org\/content\/errev\/35\/179\/250160?utm_source=25&amp;CampaignID=987&amp;interactionId=AepdEtISm_4p3L8H31st7wKCcgu0KLY5NWeOyy0DTtaJF95rc3Xk2g%3D%3D\">The Risk of Cardiac Disease Events after Respiratory Syncytial Virus Disease: A Systematic Literature Review and Meta-analysis<\/a><br \/>\nStandard review searched Embase, PubMed and grey literature sources for studies published between 1 January 2000 and 6 March 2024, reporting on cardiac events in adults with RSV disease. Study quality was assessed using a validated checklist. Absolute and relative risks of cardiac events following RSV disease were summarised and pooled estimates using random effects meta-analysis were calculated. Of 3,887 publications, 28 met the inclusion criteria. Among hospitalised patients with RSV disease (25 studies), the pooled estimates showed that 19.2% (95% CI 15.1-24.2) experienced any cardiac event (including specific and unspecific events and combinations of cardiac events), 15.7% (95% CI 14.8-16.5%) heart failure (HF) and 5.4% (95% CI 3.1-9.5%) acute coronary syndrome (ACS). Cardiac event-related mortality ranged from 1.1 to 9.8%. Compared to influenza patients, those with RSV disease had a risk ratio of 1.2 (95% CI 1.1-1.4) for any cardiac event, 1.3 (95% CI 1.1-1.6) for HF and 1.2 (95% CI 0.9-1.5) for ACS.\n\t<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0264410X26001015\">Assessing the Impact of SARS-CoV-2 Infection and Vaccination on Fertility and Assisted Reproductive Techniques Outcomes: An Umbrella Review\u00a0<\/a><br \/>\nAuthors identified 647 studies; 14 studies with 40 outcomes were included. Based on these studies it appears that COVID-19 infection may decrease semen quality in men, including semen volume (WMD, \u22120.48 ml; 95% CI, \u22120.59 to \u22120.36 ml), total sperm count (WMD, \u221234.84 \u00d7 10^6; 95% CI, \u221243.51 to \u221226.17 \u00d7 10^6), sperm concentration (WMD, \u221216.23 \u00d7 10^6\/ml; 95% CI, \u221225.56 \u00d7 10^6 to \u22126.89 \u00d7 10^6), viability (SMD, \u22120.66; 95% CI, \u22121.27 to \u22120.06), and total sperm motility (SMD, \u22120.61; 95% CI, \u22120.96 to \u22120.25), and elevated levels of estradiol (SMD 0.652; 95% CI, 0.254 to 1.049; <i>p<\/i> = 0.001) and prolactin (SMD 0.305; 95% CI, 0.045 to 0.566; <i>p<\/i> = 0.022). It did not significantly affect testosterone levels. Conversely, COVID-19 showed minimal impact on female ovarian reserve (including antral follicle count, AMH) or ART outcomes (including oocyte number and quality, embryo quality, implantation rates, clinical pregnancy rates and miscarriage rates). Vaccination also had minimal effects on both sexes. Evidence quality was generally very low, highlighting the need for high-quality, long-term studies.\n<a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/dmrr.70136\">Long-Term Risk of Incident Type 2 Diabetes Following SARS-CoV-2 Infection: A Population-Based Study in British Columbia, Canada\u00a0<\/a><br \/>\nInvestigators conducted a retrospective cohort study of all British Columbia residents aged \u2265 18 years tested for SARS-CoV-2 by reverse transcription polymerase chain reaction (RT-PCR) between 1 January 2020 and 31 January 2024. Individuals with pre-existing diabetes or residing in long-term care were excluded. Incident T2DM, identified more than 30 days after testing, was ascertained. Among 2,044,421 individuals (296,390 SARS-CoV-2-positive; 1,748,031 negative), 47,704 (2.33%) incident T2DM cases were identified over a median follow-up of 874 days. Compared with the SARS-CoV-2-negative group, the risk of diabetes was higher among infected individuals (HR 1.18; 95% CI: 1.15-1.22; <i>p<\/i> &lt; 0.0001), rising progressively with illness severity-from ambulatory to hospitalised and intensive-care cases. No evidence of association was found among the vaccinated subgroups.
